About

Defining Community Care

Community care is the term used to describe the diverse range of services provided to people who live in the community and who require community care to remain successfully living in their own home and community. Services are provided to a range of clientele including older people, younger people with disabilities, carers, people with a mental illness, people with acquired brain injury, people with chronic illness, and people with dementia. Recipients include those who have special needs including homeless people, people from culturally and linguistically diverse backgrounds and indigenous people.

Centacare Community Support Services provide service supports throughout the Archdiocese of Brisbane, and deliver services across a wide range of venues and locations, and their location often influence the types of services and support delivered.
 

Centacare Community Care Services

Community Support Services is auspiced under Centacare, providing a comprehensive range of practical support services for frail aged persons; persons with a disability; and their carers.

These community based services are designed to be specific to the needs of their particular community and have been developed through direct consultation with parishes and other interested groups.

The diverse range of services provided share the common objective of improving overall quality of life for those in the community who are ageing or living with a disability.

Community based service options include:

  • Community Aged Care Packages
     
  • Home and Community Care funded projects that provide the following:
    • Centre based respite
    • In home respite
    • Assistance with daily living skills
    • Assistance with daily living tasks
    • Recreational and educational opportunities
    • Holidays
    • Emergency support
    • Overnight/Weekend support
    • Family/Carer support
    • Transport
    • Meal provision
       
  • Home Assist Secure funded projects that provide the following:
    • Home Assist/Secure
    • Home maintenance
    • Information and advice for subsidised assistance for home or household maintenance needs
       
  • Centacareservices Carer Support Program that provides the following:

    • Flexible respite options, including overnight and emergency respite
    • Services specifically for Employed Carers of frail older people
    • Services specifically for Ageing Carers of people with a disability

The aim of our programs is to help people to be as independent as possible by providing a range of services to meet the needs of the individual who is frail and/or ageing; the younger person with a disability; and their carers.

These programs are serviced through respite centres and community based programs.
